TERMS & CONDITIONS

1. Appointments & Consultations

All appointments require a valid credit card on file. A $50 deposit is collected at the time of booking for all services except follow-up appointments, and is applied to the cost of your service. Consultations are $60: the $50 deposit is collected at booking and the $10 balance is due at your visit, with the full $60 applied to the cost of your appointment. New patients complete a consultation and intake form before treatment. We reserve the right to decline or postpone treatment at the provider's discretion.

2. Late Policy

Please arrive on time. Patients arriving more than 10 minutes late may be asked to reschedule, and the cancellation policy may apply.

3. Cancellation & No-Show Policy

We require at least 48 hours' notice to cancel or reschedule an appointment. If you cancel with less than 48 hours' notice, or miss your appointment, the $50 booking fee already collected is retained as the cancellation fee. This is not an additional charge. Repeated missed appointments may result in ending the provider–patient relationship.

4. Payment Policy

Payment is due at the time of service. We accept cash, debit cards, major credit cards, and valid gift certificates. A 3% surcharge applies to credit card transactions. There is no surcharge on debit cards.

Completed services are not refundable.

Prepaid packages. If no portion of a package has been used, it is fully refundable within 12 months of purchase. If any portion has been used, the services already received are charged at their full individual price rather than the discounted package rate, and the remaining balance is refunded.

5. Medical Disclaimer

Our services are elective, non-emergency treatments provided for aesthetic and wellness purposes. They have not been evaluated by the FDA and are not intended to diagnose, treat, cure, or prevent any disease, and do not replace care from your primary or specialty provider. Results vary. Individual treatment plans are based on anatomy, goals, and medical history. Consultation required. Patients must provide accurate and complete health information before all treatments.

6. Right to Refuse Service

We reserve the right to refuse service to any individual for any reason, including but not limited to safety concerns, inappropriate behavior, or failure to comply with our policies.

7. Photography

Clinical photography. Standardized photographs are part of your medical record. We photograph the treatment area before and after treatment to establish your baseline, plan your care, and assess how results develop over time. These images are stored in your chart in our electronic medical record, are handled with the same confidentiality as the rest of your record, and are not used outside your care.

Marketing use is separate, and it is optional. We will never use an image of you publicly without a signed marketing consent, which is a distinct document from your clinical consent and describes how the image may be used. Declining has no effect on your treatment, your scheduling, or the care you receive. You may withdraw marketing consent at any time and we will stop further use — though once material has been published or shared, we cannot always retrieve copies that others have saved or reposted.

8. Policy Updates

We may update these Terms & Conditions. Updated versions are posted here with a revision date and apply to appointments and services from that date forward.
